What does AB Mando offer?

AB Mando practices care and care development, technical development and research. All levels of care are represented at the Mandometer® Clinics at Novum and Danderyd’s Hospital: Emergency care, in-patient care, day-care, day-care visits and follow up after remission. We also engage in preventive care in the form of educational visits and lectures.
 

Remission criteria

  • Normal eating behavior and satiety
  • Normal body weight
  • Normal psychiatric profile
  • Normal laboratory test values
  • The patient her/himself shall be able to say that food and weight are not a problem any longer. 
  • The patient shall be back to school or work and will be engaged in leisure activities
  • No binge-eating and vomiting over the last three months for patients with bulimia.

AB Mando has two clinics in Stockholm, Sweden, one at Novum next to the Karolinska University Hospital in Huddinge, and one at Danderyd’s Hospital in north Stockholm. In January 2010 a new Mandometer® clinic opened in Alingsås, Sweden. We also have a general agreement with the counties of Gävleborg and Värmland for treatment at the Mandometer® Clinic at Novum.

We have a Mandometer® clinic in New York City, USA, and one in Melbourne, Australia. 

Finally, we have started two clinics for overweight and obesity, the Mandolean® Clinics, in the Novum Research Park and at Danderyd’s Hospital, and one in New York City.
 

Opening hours

Mandometer® Clinic Novum is open 24/7 365 days per year.
Mandometer® Clinic Danderyd´s Hospital is open 8 am – 7 pm, five days a week.
Mandometer® Clinic Melbourne is open 8 am – 6.30 pm, five days a week. 

 

What care can you get?

The different types of care are: emergency care, in-patient care, day-care with accommodation in patient apartments or patient hotel, outpatient care and follow-up after remission.
 

Personnel and competence 

Our staff consists of behavioral scientists, doctors, nurses, assistant nurses, dieticians and nutritionists. AB Mando is manned around the clock by doctors, nurses and assistant nurses. The emergency doctor support staffs consist of eight doctors with the following specialities: Internal medicine, paediatricians and family medicine. In addition we have a psychiatrist on call.

 

All treatment personnel at AB Mando go through a certification in the treatment concept before they start working independently at the clinic. A structured teaching program consisting of eight study units. The training involves giving the participants a thorough survey of theory and practice within the eating disorder area, as well as knowledge of AB Mando’s specific treatment concept. All case managers who are newly employed shall have a university degree in behavioural science. The majority of the case managers have a master’s degree.
 

A well-functioning organization demands good leadership and therefore AB Mando have invested a great deal in their leaders. Present and potential managers participate continually in a development programme for managers and in a weekly management meeting with the aim of continual improvement.

 

The number of occupied beds

We have an in-patient ward comprising 12 beds, one of which is reserved for emergency patients. On average, occupation is 85-90%. The average in-patient stay is more than 30 days. There is a good cooperation between AB Mando and the Karolinska University Hospital concerning these patients. Today, we have more than 160 patients in treatment. 
 

Number of patients in remission

To date, more than 750 patients have gone into remission. We follow these patients in a follow-up program for five years. The relapse rate is approximately 10 %. 
 

Diagnoses and level of severity

We treat all levels of severity and the diagnoses anorexia, bulimia and unspecified eating disorder, EDNOS. We only exclude patients with alcohol and/or drug dependence.
 

Age and gender

We treat all ages and both sexes.

Services provided

Emergency care

Eating disorder patients with the following symptoms are hospitalized immediately:
 
1) Pulse <35-40 beats/min and/or bodytemp< 35.5 - 36 0 C,
2) Low potassium: K< 2.5-3.0 mmol/l,
3) Low phosphate: P < 0.65 mmol/l,
4) Low magnesium: Mg<0.7 mmol/l,
5) EKG –changes with prolonged QTc interval > 450-500 msec
6) Serious dehydration
7) Risk of suicide
 
The care of patients with severe eating disorders does not function optimally in the regular medical wards, where there is a fear of this patient group. Patients with eating disorders often create anxiety and worry among personnel and other patients. Furthermore, the regular health care clinic does not have the resources or knowledge to be able to treat the patient after the urgent problem is over. This means that many patients are sent home with no treatment and the urgent problems almost invariably recur.
 

The in-patient care at AB Mando is integrated with behavioural therapy that makes it possible to offer the patient specialized, expert care directly. Often, a patient has a high motivation to initiate treatment just after they experience a serious health event resulting from their eating disorder and it is imperative to initiate the full treatment under those conditions for optimal outcomes. 

 

Eating Evaluation

All patients seeking care at a Mandometer® Clinic undergo an eating evaluation, which takes about 5 hours, including the analysis of the results. The evaluation is a validated diagnostic instrument used to make a correct diagnosis, determine the degree of severity, and exclude other diagnoses. Laboratory tests include complete blood status, nutrition tests and malabsorbation tests. The patient meets a physician, a case manager and a nurse.

 

In-patient Care

Care is provided 24/7. The 24-hour care includes medical service, as well as medical/psychiatric examinations. Laboratory analyses are included. During day, the patient receives eating training, thermal treatment, activity modification and social rehabilitation. The patient is treated by a physician, a nurse and a case manager. 

 

Partial Hospitalization

Day care is provided with more than 3 hours(see below) and sleeping accommodations. The care includes medical service, as well as medical/psychiatric examinations. Laboratory analyses are included. During day, the patient receives eating training, thermal treatment, activity modification and social rehabilitation. The patient is treated by a physician, a nurse and a case manager. 

 

Caregivers eat with patients after the ordinary day care is closed. The patients have access to the caregivers around the clock in partial hospitalization. The proximity of the partial hospitalization brings a sense of security to patients and their relatives. The progress of all patients in partial hospitalization is discussed during daily rounds. Parents or other relatives live together with the patient. AB Mando has fourteen rooms with a common kitchen and public room next to the inpatient ward at Novum. Moreover. there are 9 patient apartments in Flemingsberg’s centre, next to the clinic.

 

Day care (more than 3 hours)

Care is provided more than three hours/day, but not necessarily 7 days/week. The care includes medical service, as well as medical/psychiatric examinations. Laboratory analyses are included. During the daytime, the patient receives eating training, thermal treatment, activity modification and social rehabilitation. The patient is treated by a physician, a nurse and a case manager. 

 

Out-patient visit (less than 3 hours)

During the day, the patient receives eating training, thermal treatment, activity modification and social rehabilitation. The patient is treated by a physician, a nurse and a case manager. This service can be combined, during the same day, with one or several “other visits” and/or one or several physician’s visits.

 

Physician’s visit

The care includes medical/psychiatric examinations made by a physician. Laboratory analyses are included. The patient is treated by a physician. This service can be combined, during the same day, with one or several “other visits” and/or an out-patient visit (less than 3 hours).

 

Other visits

Any other visit including phone visits. The patient is treated by a physician, a nurse or a case manager. This service can be combined in the same day with an outpatient visit (less than 3 hours) and/or a physician’s visit.

 

Follow-up

More than 750 patients have been treated until remission since the first patient was treated in 1993. The patients are followed for five years and the relapse frequency is extraordinarily low, approximately 10 %. That is far less than in previous reports (30-70% within a year Pike et al. Am J Psychiatry 2003;160: 2046). All diagnoses respond equally well to our treatment. Those few patients who relapse are offered new treatment and are usually easier to treat to remission the second time.

There are no other treatment centres that have the strict remission criteria that we use in our clinics. 

 

Referrals to the Mandometer® Clinic

We wish to get information about age, weight, length, including growth charts for patients 18 years of age and younger, somatic status, including pulse, menstruation and current medicines. Presumed eating disorder diagnosis, AN, BN or EDNOS, eating disorder duration, weight loss over the last three months, presence of binge-eating, vomiting, hyperactivity and suicidal risk. The referral shall state that the Mandometer® method has been actively chosen.
